I, undersigned, hereby acknowledge that I have voluntarily applied to participate in glass blowing / flame working activities which will involve actual handling of glass working equipment and hot glass, in or around an open flame and kilns.
I AM AWARE THAT GLASS BLOWING INSTRUCTION AND TRAINING IS A HAZARDOUS ACTIVITY, AND I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED AND HEREBY AGREE TO ACCEPT ANY AND ALL RISK OF INJURY OR DEATH.
As lawful consideration for being permitted by Ignite Glass LLC (“Ignite Glass Studios”) to participate in this instruction and training and use of its Facilities, I hereby agree that I, my heirs, distributes, guardians, legal representatives and assigns will not make claim against, sue, attach the property of, or prosecute Ignite Glass Studios, or its owners, employees, successors, heirs and assigns, for injury, death or property damages resulting from my participation in activities at Ignite Glass Studios, including but not limited to, the negligence or other acts, howsoever caused, of any employees, agent, owner, other students, or contractor of Ignite Glass Studios, its successors or assigns. In addition, I hereby release and discharge Ignite Glass Studios from any and all actions, claims or demands that I, my heirs, distributes, guardians, legal representatives or assigns now have or may hereafter have for injury or damage resulting from my participation in glass blowing instruction or training. Ignite Glass Studios reserves the right to photograph and or audio/video record your glassblowing participation for promotional purposes. By signing this waiver, I hereby acknowledge that any consumption of alcohol is at my own discretion, and I assume all risk involved with the consumption of alcohol and unconditionally hold the Ignite Glass Studios harmless.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND IGNITE GLASS STUDIOS, AND OR ITS OWNERS, EMPLOYEES AND CONTRACTORS, AND I HAVE SIGNED IT OF MY OWN FREE WILL.
